REQUEST BILLING RECORDS
Boulder Valley Anesthesiology does not maintain patient medical records. All medical records must be requested directly from the facility where your procedure or surgery took place, as they are the official custodian of those documents.
We are happy to assist with any billing records requests.
To submit a request for billing records
We fulfill third-party billing record requests electronically via ChartSwap, a HIPAA-compliant platform that facilitates secure medical and billing record exchange. As a reminder our billing partner, Coronis Health, does not have medical records.

ChartSwap allows requestors to:
· Register for free
· Submit, track, and manage record requests
· Pay invoices and download records—all in one place
To submit a record request, follow these steps:
1. Visit https://www.chartswap.com/register to register as a Record Requestor
2. Use the ChartSwap portal to search for providers, enter request details, and upload supporting documents
3. Your request will be reviewed, and you’ll receive a notification once your records are available
4. Pay your invoice via check, credit, or debit card directly in the ChartSwap portal
